This is not exclusive for these diagnoses but far more prevalent in them as a baseline. You can get the same effect with heavy fatigue or stress.

You can be very smart and educated and STILL struggle with expressing those thoughts. It doesn’t make you stupid or thoughtless, it means that your brain has difficulties expressing itself. Usually what researchers express is one of the major issues here is working memory. It’s like the buffer state between you thinking it and that information getting to your mouth having a really bad connection and can’t manage big chunks of information, like a slow buffer on a video, so everything comes out garbled. It the same reason you can mess up a sentence by having sounds switch places at the start and end of a sentence.

Generally, it means that making tasks smaller and shorter helps, as well as relaxing. Working memory is tied to a lot of things, from speech and concentration to short term memory and general structuring of tasks. Executive disorders are very much tied to working memory. It really doesn’t mean you’re stupid, but society does equate the ability to be expressive with intelligence. It’s a false equivalence.
